		Insurance & Billing: Network Participation : Medical Network, Private Healthcare Systems and One Health Plan. Hospital Affiliations: Maine Medical Center, Mercy Hospital, Southern Maine Medical Center Payment policies: The payment of medical services can be a confusing and troubling experience. While no one denies the right of a medical practice to receive reimbursement for its services, it can be, on the other hand, an overwhelming problem when a patient is confronted with large medical bills. Consequently, the problem of paying for medical services is sometimes avoided as a result of feeling as though there are few options. At Maxwell, Kluger & Makaretz, ENT Associates, MD, PA, we want to work with our patients to make payment arrangements that are reasonable and attainable. Patients are strongly encouraged to speak with our billing department any time there is a question or concern with our charges or with balances owed to the practice. Our policy provides for all patients to be given the opportunity to pay for their medical services under an arrangement that is within their budget and fair to the medical practice. We will bill to all major insurance and third party payers. We will verify with your insurance company if a deductible or co-insurance applies to the services being provided. Patients will be asked to bring payment for deductibles and co-insurance to their pre-operative appointment before all surgeries and major procedures. If the patient is unable to make this payment in full, we will gladly work out an acceptable payment arrangement. We will communicate with those patients who have deductibles and co-insurance before the pre-operative appointment. Patients will be expected to pay for any co-payments at the time of service as may be allowed under their health plan. Patients are asked to provide current and complete insurance information before or on the day we provide services. Patient registration and history forms can be downloaded from this site for completion to bring or mail to the office.
